Request for the EU-ACME accreditation

If a country has NO CME system at all and NO accreditation institution/committee, a CME provider may apply for EU-ACME accreditation online.

Documents templates:

  1. Director's declaration to be completed and signed by the medical practitioner who will take responsibility for the application
  2. Conflict of Interest to be completed and signed by chairman of scientific and/or organising committee and members of a committee
  3. Conflict of Interest to be completed and signed by members of scientific and/or organising committee
  4. Template Evaluation Form completed by participants
  5. Template Event Report
